Child must be 4 years old by September 1, 2015

Qualify for free/reduced lunch or currently receive Medicaid

 

Information needed to register your child:

 

Proof of residency in Cheraw attendance area

Birth Certificate

Immunization

Social Security card

Medicaid Card

Proof of Income*
